What we do
Ty Hafan provides specialist palliative care to life limited children and their families. We provide this care for children and young people up to the age of 18.
Specialist palliative care may include end of life care, bereavement care, short break care, emotional support and outreach services. Ty Hafan provides this support to children and families across Wales.

- Can provide specialist support for families of children with disabilities / additional needs
Service caters for children with severe disabilities and complex health needs whose conditions mean it is likely they will not survive into adulthood. Please contact to discuss your child(s) needs further. - Relevant staff have a valid current DBS?
