Carmarthenshire Team Around the Family (TAF) - Family support services


What we do

TAF is a way of working that brings agencies together to deliver a plan of support to a family, child or young person up to the age of 25. All Families First projects work with a TAF approach. The Joint Assessment Family Framework (JAFF) – a standardised assessment tool is completed with families. It is used for planning actions and to demonstrate improved outcomes. A TAF Key Worker from any project can take the lead role, working with families so that services are co-ordinated and meet the family’s needs. The TAF Team facilitate this approach:
• TAF Co-ordinators allocate or facilitate a package of support.
• TAF Direct Workers work directly with families towards an identified outcome.

Parents can self refer via online referral form, referrals are also accepted from GP's and all professional agencies

Language: Welsh medium setting and English medium setting

  • Can provide specialist support for families of children with disabilities / additional needs Yes Please contact to discuss your child(s) needs further. TAF work closely with partner agencies who can provide ALN support. TAF also support families with children and young people with Neurological health issues, providing information workshops for parents and 1:1 support.
    TAF are unable to assess and diagnose health conditions or expedite medical assessments.
