What we do

Children, young people and their families sometimes need a little extra support for them to have happy and healthy lives. Team Around the Family (TAF) is an early intervention and prevention service that aims to work with families to help them identify their strengths and needs. TAF will then support them to achieve their individual goals by offering advice, guidance and support. TAF can also support families through the co-ordination of other agencies.
• TAF provides a service to families who are likely to need help from two or more agencies (e.g. schools health services and housing)
• TAF brings everyone together with families to identify strengths and any needs, and how best to enable families to meet those needs
• TAF is a way of identifying who is best placed to offer any of the additional advice and support your family may need.

Who do we support

Families with children aged 0-25 years in Neath Port Talbot.

We provide a service to families where:
• There is a family member 0-25 yrs
• Difficulties have been identified that require help from 2 or more agencies e.g. school, health, housing.
• There is no social services involvement
You can be referred to TAF by another professional that knows your family such as a Teacher, GP, Health Visitor, Youth Worker or you can refer yourself.
The aim is to support families to make the changes needed to improve their lives and live independently within their own communities.

Is there a charge to use this service?

No

Can anyone use this service?

A referral will be needed for all services and families can refer themselves or be referred by someone who knows the family, like a teacher or Health Visitor. Anyone making a referral for a family must talk to the family about it so that they know what information is being given and the type of support that is being requested. Referrals for all services can be made through the Single Point of Contact (SPOC) and are available to families who do not need support from Social Services.
