Supporting Family Change - Family support services
What we do
Supporting Family Change (SFC) coordinates support for families in times of need.
The project brings together a Team Around the Family (TAF) who can offer support for a wide range of needs, particularly when a family has more than two needs.
A designated support practitioner works with the family, along with other
agencies to develop a multi-agency plan that focuses on the families’
strengths. This helps to break the cycle of any repeated needs, providing opportunities to access other support interventions and build resilience

Families with children under the age of 25 years.
