Ceredigion Pathfinder Pilot Programme - Family support services


What we do

The aims of the Pathfinder Pilot Programme will be:

- To build both individual and community resilience to enable families to live safe, healthy and fulfilling lives where they can successfully raise their children and fully optimise their potential.
- To achieve this we will transform the delivery of Early Years Services from those that are segmented, and aligned along statutory boundaries, to those that use a placed based approach. This approach will not only provide integrated services but will also use a strengths based model to improve relationships between sectors, improve community engagement and build safe and supportive environments.

Who do we support

Welsh Government have invested in a range of programmes including Healthy Child Wales, Foundation Phase and Flying Start to ensure our families with children aged 0-7 years have access to the right support at the right time.

Integrated support for children aged 0-7 and their families living in all postcodes in the Pathfinder areas:

Lampeter
Tregaron

Can anyone use this service?

Families living in the Pathfinder areas will be able to access support from the Pathfinder multi-agency staff – Family Centre staff, Midwives, Health Visitors, Family Support Workers, Speech and Language Therapists, Childcare staff, Schools, GP Surgeries, Employment agencies and other agencies.
