Disability Wellbeing Navigator - Family support services

What we do

The disability navigator role will give me the opportunity to be Denbighshire County Council’s key point of contact for children and their families. I am hoping it will help them access high quality, appropriate information, advice and assistance. This will enable them to meet their personal wellbeing goals and outcomes.

Who do we support

I will be navigating and supporting children, young people and their families with disabilities across the local authority and ensuring that children are supported throughout the year and through any transitions.
I will also be working alongside education services and will help to contribute and facilitate engagement of families with children who have more complex needs.
